Insurance & Superbills

Crystal Cove Wellness currently accepts select Blue Cross Blue Shield and Cigna insurance plans for eligible patients throughout California. We also welcome self-pay patients. For patients using out-of-network benefits, superbills are available upon request.

A superbill is an itemized receipt containing the clinical information your insurer may require — including diagnosis codes, procedure codes, and provider credentials.

Many California insurance plans include out-of-network mental health benefits. We encourage you to contact your carrier to ask about your out-of-network coverage and reimbursement rates prior to your first appointment.

HSA & FSA Eligible

Mental health services are generally eligible for payment using Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA). Please confirm eligibility with your plan administrator.

Telehealth Parity in California

California law requires many health plans to cover telehealth services at parity with in-person care. Contact your insurance carrier to ask about your out-of-network mental health benefits.

Superbill Available Upon Request

Superbills (itemized receipts with clinical codes) are available upon request so patients can submit for potential out-of-network reimbursement from their insurance carrier.
